fix(build): 重生成 Windows 图标补齐 64/96/128 中间尺寸层

原 icon.ico 仅含 16/24/32/48/256 五层，高 DPI（如 4K 200% 缩放）任务栏
需要约 64px 图标时只能从 48 放大导致模糊。现补齐 16-128 全梯度 BMP 层
+ 256 PNG 层，NSIS 安装器图标一并受益。
